  • Internal Voucher (please confirm with your admin prior to selecting this option)

  • No payment (ONLY for presenters, interpreters & planning committee)

Family Registration

Step one:

Fill out the registration form by clicking HERE

Pricing: July 18 - September 4 (Flat Fee for 1 or 2 days: $50)

This cost will cover hotel accommodations for up to 2 nights, all meals, childcare and workshops

A $25 stipend is available for families who need financial assistance

  2. DeafBlind Child Special Needs - Information Form

  3. Sibling Special Needs - Information Form (please complete one form for each sibling attending)

  4. Parent/Guardian Informed Consent for Video/Photo Form

  1. Formulario de Exencion de Responsabilidad Para El Cuidado Infantil 

  2. Formulario de Informacion Sobre Las Necesidades Especiales Del Nino Sordociego 

  3. Formulario de Informacion Sobre Las Necesidades Especiales de Los Hermanos (por favor, complete un formulario por cada hermano que asista)

  4. Consentimiento Informado del Padre, Madre, o Tutor Para Fotografias y Grabaciones en Video

The fee is charged per agency (not per attendee), so there is no additional cost if more than one representative attends.
